Home Health care & Hospice

    ProHealth Home Care provides part-time intermittent skilled services in the home for patients who require short-time assistance with the management of their medical condition. Services include skilled nursing, rehabilitation therapy, social work and personal care.

    ProHealth Home Hospice provides care in the home for patients with a terminal illness, emphasizing the quality of remaining life. Support is provided by a multidisciplinary team that includes physicians, nurses, social workers, chaplains, trained volunteers and home health aides. Hospice also provides a bereavement program for families for one year following the death of the patient.

Residential Hospice
